The application submission period for DV-2005 is from 00:00 1 November 2003 (EST) to 24:00 30 December 2003 (EST). The application form will only be available for submission during this period and this period only. Applications will not be accepted through the U.S. Postal Service.

Application Form
Formular na vyplnenie prihlasky:

Formular ma dve casti.  Prva cast je pre vas, druha cast je pre vase deti a manzelku.  The application form is prepared in two parts, one for the primary applicant and one for dependents (if any). Each part must be completed correctly and completely before going to the next part. Kazdu cast musite celu vyplnit predtym ako zacnete vyplnovat dalsiu cast.  After all parts are completed, the application will be submitted to the DV-2005 Processing Center for handling.
